Bloom Healthcare Expands Dallas-Fort Worth Presence with Acquisition of Christian Care House Calls

Acquisition brings Christian Care House Calls providers into Bloom Healthcare and expands access to high-quality home-based primary care

DENVER, Aug. 31, 2026 (GLOBE NEWSWIRE)Bloom Healthcare, a passionate team of medical providers specializing in in-home primary care for seniors with complex health needs, today announced its acquisition of Christian Care House Calls, a Dallas-Fort Worth-based home primary care practice. Effective August 31, Christian Care House Calls will operate under the Bloom Healthcare name, strengthening Bloom’s presence in the region and expanding its ability to provide high-quality primary care to patients in their homes and senior living communities.

The acquisition brings together two organizations with a shared commitment to compassionate, patient-centered care delivered where patients live. Christian Care House Calls providers will join the Bloom Healthcare team and continue caring for their existing patients, providing continuity of care as the practice transitions to the Bloom Healthcare name.

“Christian Care House Calls has built trusted relationships with patients, families, and communities across Dallas-Fort Worth for more than 10 years, and we are thrilled to welcome its providers and patients to Bloom Healthcare,” said Thomas Lally, MD, founder and CEO of Bloom Healthcare. “This acquisition allows us to build on that strong foundation while expanding the resources and support available to patients throughout the region. Together, we can help more seniors receive consistent, coordinated care in the place they are most comfortable: home.”

Bloom Healthcare has provided home-based primary care for more than two decades, building a model designed around the needs of seniors with complex health conditions and the families and caregivers who support them. Its multidisciplinary approach connects primary care providers with nurse clinical coordinators, social workers, pharmacists, behavioral health specialists, and wound care specialists to deliver care according to a comprehensive plan developed with patients and their families. Through Bloom’s integrated Hospice, that coordinated approach can continue through end-of-life care, with individualized care plans that honor the wishes of patients and families and keep primary care closely involved throughout the care journey.

Bloom Healthcare’s services include in-home primary care, chronic care management and preventive care, dementia care planning and caregiver support, behavioral health and geriatric psychiatric services, and coordinated care across healthcare providers and settings, as well as integrated Hospice. Patients also have 24/7 telephone access to on-call providers for urgent needs.

The addition of Christian Care House Calls strengthens Bloom Healthcare’s growing presence in Dallas-Fort Worth and extends its relationships with patients, families, senior living communities, healthcare organizations, and referral partners across the region. As an Accountable Care Organization (ACO), Bloom uses a population health approach focused on quality, coordinated care, and helping seniors spend more days healthy at home. The partnership will also extend the reach of the Bloom Flourish Thrive Foundation, the organization’s charitable arm dedicated to helping older adults access the resources they need to stay healthy, safe, and independent at home.
